We aim to make the referral process as straightforward and responsive as possible. To submit a referral, please provide:

  • Young person’s age and background / Care plans / reports
  • Presenting needs and risks
  • Required placement type
  • Any immediate safeguarding concerns
  • Preferred location (if applicable)
  • Relevant assessment or supporting documents

Referrals can be made via email, phone, or through a secure referral form.

What Happens Next

Once we receive your referral:

  1. Immediate Review
    Our team assesses the information and identifies the most suitable placement or support package.
  2. Matching & Risk Assessment
    We match the young person’s needs with the right home, team or crisis response service.
  3. Decision & Confirmation
    A placement decision is communicated promptly, with clear details on staffing, model of support and next steps.
  4. Transition & Move-In
    We coordinate arrival, provide welcome information, and implement placement plans immediately.

We ensure full transparency throughout the process and provide all required documentation to support multidisciplinary decision-making.
