We aim to make the referral process as straightforward and responsive as possible. To submit a referral, please provide:
- Young person’s age and background / Care plans / reports
- Presenting needs and risks
- Required placement type
- Any immediate safeguarding concerns
- Preferred location (if applicable)
- Relevant assessment or supporting documents

What Happens Next
Once we receive your referral:
- Immediate Review
Our team assesses the information and identifies the most suitable placement or support package. - Matching & Risk Assessment
We match the young person’s needs with the right home, team or crisis response service. - Decision & Confirmation
A placement decision is communicated promptly, with clear details on staffing, model of support and next steps. - Transition & Move-In
We coordinate arrival, provide welcome information, and implement placement plans immediately.
We ensure full transparency throughout the process and provide all required documentation to support multidisciplinary decision-making.
